CONSULTATIVE GERIATRIC ASSESSMENT SERVICE

I provide comprehensive geriatric assessments for residents of residential aged care facilities, with a focus on the evaluation and management of complex geriatric conditions.

The service may include:

Comprehensive medical review and assessment of complex medical conditions
Frailty assessment
Medication review and optimisation
Cognitive assessment and evaluation of dementia
Falls risk and balance assessment
Assessment of functional status and other geriatric syndromes
Recommendations regarding ongoing management and care planning

This is a consultative service and does not constitute a transfer or takeover of ongoing medical care. The treating General Practitioner (GP) and/or Nurse Practitioner (NP) remains responsible for the patient's ongoing clinical management and care.
